UHC plan types to know
Choice and Choice Plus — the most common employer products. Choice Plus includes out-of network benefits; Choice generally does not.
Options PPO — broad network, no PCP requirement.
Navigate and Navigate Plus — these require a designated primary care physician and referrals for specialists. If you have a Navigate plan, you must name Dr. Thomas as your child's PCP or claims may deny.
Charter and Core — narrower network products; verify specifically.
UnitedHealthcare Community Plan — this is UHC's Medicaid product and operates under different rules.
Designating a PCP
For Navigate plans (and recommended for all plans), designate Dr. Priya Thomas or Dr. Swati Mahajan as your child's primary care physician:
-
Log into myuhc.com or the UnitedHealthcare app
-
Or call member services on the back of your card
You'll need our practice information and Dr. Thomas's/Dr. Mahajan's NPI — call (770) 746-7113 and we'll provide it
What's typically covered
Most UHC plans cover pediatric preventive care at 100%: well-child visits, routine immunizations, developmental screening, and vision/hearing screening.
Sick visits and procedures are subject to copay, coinsurance, or deductible. UHC's high-deductible plans paired with HSAs are very common on employer plans in this area — check your deductible status in the myuhc app before a non-preventive visit.
Useful tip: the UnitedHealthcare app shows real-time deductible and out-of-pocket status, which is worth checking before any sick visit so the bill isn't a surprise.
Newborns
Add your baby to your UHC policy within 30 days of birth. Coverage is typically retroactive to birth once enrolled. We see newborns 1–2 days after hospital discharge
